After completing your new patient form, our Virtual Connection Team will contact you with a link to our provider schedule, allowing you to select a time that works best for you. To prepare for your first appointment, please download the Athena app and access the patient portal to complete the required paperwork.
How does the intake process work?
After you submit your intake forms, we meticulously build your chart for the provider and check your insurance details. We understand that dealing with insurance can be daunting, but our team is dedicated to simplifying the process for you. We may require some patients to create an account with Alma for billing purposes and request auto-pay setup for co-pays as per policy.
